HoLEP Prostate Surgery in London

HoLEP is a laser procedure for urinary symptoms caused by an enlarged prostate. It removes the prostate tissue that is blocking urine flow and may be suitable for men with larger prostates, as well as those seeking a durable treatment option.

Our consultant urologists can assess whether HoLEP is appropriate for you, explain how it compares with other prostate procedures and guide you through treatment and recovery.

What is HoLEP?

HoLEP stands for holmium laser enucleation of the prostate. It is a minimally invasive operation for benign prostatic hyperplasia, also known as BPH or an enlarged prostate.

As the prostate grows, it can narrow the urethra, the tube that carries urine from the bladder out of the body. This can cause symptoms such as a weak urine flow, difficulty starting, straining, frequent trips to the toilet or waking at night to pass urine.

During HoLEP, a holmium laser is passed through the urethra to separate the inner part of the enlarged prostate from its outer capsule. The tissue causing the blockage is then broken into smaller pieces and removed through the bladder.

As HoLEP removes the part of the prostate causing the obstruction, it can provide a long-lasting improvement in urine flow for suitable men. It may be considered across a range of prostate sizes, including larger glands that may not be suitable for some other procedures.

The operation is performed under anaesthetic. A catheter is usually needed for a short period afterwards. As with other procedures that remove prostate tissue, there can be risks and side effects, including changes to ejaculation. Your consultant will discuss these in relation to your circumstances.

How it works

Separating and removing obstructing prostate tissue

HoLEP treats bladder outlet obstruction by removing the inner prostate tissue that is narrowing the urine channel.

Laser enucleation

A holmium laser separates the obstructing inner prostate tissue from the outer part of the prostate.

Tissue removal

The separated tissue is broken into smaller pieces and removed through the bladder during the same operation.

A durable treatment approach

By removing the tissue causing obstruction, HoLEP can provide a lasting treatment option. Individual outcomes and the need for any further treatment vary.

Who HoLEP surgery may suit

HoLEP may be considered for men who:

  • Have troublesome urinary symptoms caused by an enlarged prostate
  • Have a larger prostate or a prostate size suitable for complete tissue removal
  • Have not had enough improvement with lifestyle measures or medication
  • Would like to discuss a durable surgical treatment option
  • Understand that prostate surgery can affect ejaculation
  • Have been assessed as suitable by a consultant urologist

Frequently asked questions

Is HoLEP suitable for a large prostate?

HoLEP may be suitable for men with larger prostates, as well as for other prostate sizes. Your consultant will assess the size and shape of your prostate and discuss whether it is appropriate for you.

How long do HoLEP results last?

HoLEP removes the prostate tissue causing obstruction, so it is considered a durable treatment approach. However, individual outcomes vary and your consultant will discuss what you can reasonably expect.

Will HoLEP affect ejaculation?

HoLEP can cause retrograde ejaculation, where semen travels backwards into the bladder during orgasm rather than leaving through the penis. This is not harmful but can affect fertility. Your consultant will explain this and any other possible effects on sexual function before surgery.

Will I need to stay in hospital?

A catheter is usually required for a short period after HoLEP. Some men may need a short hospital stay, depending on their health, the procedure and their recovery. Your consultant will discuss what applies to you.

Is HoLEP better than TURP?

Neither procedure is right for every man. HoLEP may be particularly appropriate for larger prostates, while TURP remains a suitable treatment for many men. Your consultant will explain which options best match your prostate, symptoms and priorities.

How long does it take to recover from HoLEP?

Recovery differs between individuals. You may have temporary urinary symptoms, such as increased frequency or mild burning, while the area heals. Your consultant will provide specific advice about recovery, activity, work and follow-up.
