What is a tight foreskin (phimosis)?

Phimosis is the medical term for a foreskin that is too tight to pull back fully over the head of the penis.

It is normal for the foreskin to be non-retractable in young boys and it often becomes more flexible naturally over time. In adults, a tight foreskin can sometimes cause symptoms or need treatment.

There are several possible reasons why phimosis develops. Recurrent inflammation or infection can lead to swelling and scarring. Skin conditions, including lichen sclerosus, can also affect the foreskin. Some men notice pain or tightness during erections, redness or soreness, difficulty cleaning beneath the foreskin or discomfort during sex.

A tight foreskin may also cause ballooning when passing urine or affect the direction of the urine stream. A planned consultation can help establish what is causing the problem and which treatment may help.

Common symptoms of phimosis

Recognising it

You may experience one or more of the following symptoms.

Tightness or discomfort

Difficulty pulling the foreskin back, or pain and tightness during erections.

Soreness and inflammation

Redness, swelling, itching or recurrent irritation around the foreskin or head of the penis.

Hygiene and urinary problems

Difficulty cleaning under the foreskin, ballooning during urination or a spraying urine stream.

Discomfort during sex

Pain during sexual activity or tearing of the foreskin.

When a tight foreskin needs urgent care
Paraphimosis is different from phimosis. It happens when the foreskin has been pulled back but becomes trapped behind the head of the penis and cannot be moved forward again. This can cause swelling and pain and needs urgent medical attention. If your foreskin is stuck behind the head of the penis, seek urgent medical care.

How we assess phimosis

A consultation and discreet examination are often enough to establish the degree of tightness and plan the appropriate treatment.

Examination

Your consultant will assess the foreskin and look for signs of inflammation, scarring or an underlying skin condition.

Swabs or tests where needed

If infection or a skin condition is suspected, simple tests may be recommended to help guide treatment.

Assessment of symptoms

Your consultant will discuss any difficulties with hygiene, urination, erections or sexual activity, and how symptoms are affecting you.

Treatment options

Conservative treatment

Steroid cream and gentle stretching

For some men, a prescribed steroid cream and a gradual stretching programme can help improve flexibility of the foreskin. Your consultant will advise whether this approach is appropriate and how to use treatment safely.

Surgical treatment

Circumcision

Circumcision is a procedure to remove the foreskin. It may be recommended where phimosis is persistent, severe, linked to scarring or caused by an underlying skin condition. Your consultant will explain what the procedure involves and whether it is the right option for you.

From assessment to treatment

Treatment is guided by the cause and severity of phimosis. Milder cases may improve with conservative treatment, including prescribed steroid cream, gentle stretching and advice on hygiene.

If there is significant scarring, recurrent problems or symptoms that have not improved with non-surgical treatment, a procedure may be considered. Your consultant will explain the potential benefits, risks and practical considerations of each suitable option.

Frequently asked questions

Is a tight foreskin serious?

Phimosis is usually not serious, but it can cause discomfort, hygiene difficulties, recurrent inflammation or pain during sex. In some cases, it may be associated with a skin condition that needs treatment. Assessment can help clarify the cause.

Will I always need circumcision?

No. Many cases can improve with prescribed steroid cream and gentle stretching. Circumcision may be considered when non-surgical treatment has not helped, or where there is significant scarring or an underlying skin condition.

Is the assessment embarrassing?

It is understandable to feel apprehensive about an intimate examination. Our consultants handle this common concern respectfully and discreetly, and will explain what the assessment involves beforehand.

What is paraphimosis?

Paraphimosis occurs when a foreskin that has been pulled back becomes trapped behind the head of the penis and cannot be returned to its normal position. It requires urgent medical attention.

Do I need a GP referral?

No. You are welcome to self-refer and arrange a consultation directly with our team.
