What Is Vestibular Testing?

The vestibular system, located in the inner ear, works closely with your eyes and brain to help maintain stability and spatial awareness. When this system is not functioning properly, it can cause spinning sensations, unsteadiness or nausea.
Vestibular testing evaluates how well the balance organs of the inner ear and their nerve connections are working. These tests measure eye movements and balance responses because the inner ear and eye muscles are closely linked.
If there is a problem in one ear or within the balance pathway to the brain, subtle changes in eye movement patterns can help pinpoint the issue. Vestibular testing provides objective information that supports diagnosis and treatment planning.


When Is Vestibular Testing Recommended?
Your clinician may recommend balance testing if you experience:
- Recurrent vertigo or spinning sensations
- Persistent dizziness or light-headedness

What Tests May Be Included?
A vestibular assessment may include several components depending on your symptoms.
Videonystagmography (VNG Test)
A VNG test records eye movements using infrared cameras while you follow visual targets or change head positions. Abnormal eye movements can indicate imbalance within the inner ear or its neural pathways.
Caloric Testing
Caloric testing gently stimulates each ear separately using warm and cool air. This allows the audiologist to compare balance function between the left and right sides. Differences can help identify weakness or asymmetry in the vestibular system.
Positional Testing
Positional testing involves moving your head and body into specific positions to see whether symptoms are triggered. This is commonly used to diagnose benign paroxysmal positional vertigo, also known as BPPV.
Additional elements may be included where clinically appropriate to ensure a thorough vestibular function test.
What to Expect During Your Appointment
Your appointment begins with a detailed discussion of your symptoms, medical history and any previous episodes of dizziness. The audiologist will explain each stage of testing clearly before it is performed.
Vestibular tests are non-invasive and generally well tolerated. Some parts of the assessment may briefly reproduce dizziness, but this is temporary and carefully monitored. Results are usually discussed on the same day with advice on next steps or referral if needed.

- How long does vestibular testing take?
A full vestibular assessment typically takes between 45 and 90 minutes depending on the number of tests required. Your audiologist will explain the expected duration at the start of your appointment.
- Is vestibular testing uncomfortable?
The tests are safe and non-invasive. Some procedures may briefly trigger dizziness, but this settles quickly and is part of understanding how your balance system responds.
- Do I need to prepare for a VNG test or caloric testing?
You may be advised to avoid certain medications before testing and to attend without eye makeup if VNG is planned. Full preparation instructions will be provided in advance of your visit.
- Can children have vestibular testing?
Yes. Vestibular testing can be adapted for children depending on age and ability to follow instructions. A specialist audiologist will determine the most suitable approach.
