Alkaline Phosphatase Test

An alkaline phosphatase (ALP) test is a routine examination often included as part of a broader blood panel. It measures the level of alkaline phosphatase – an enzyme found in the liver, bones and other tissues. This test helps evaluate liver function and bone health, providing valuable insight into a variety of medical conditions. Depending on the results, further investigations may be recommended to pinpoint the underlying cause of abnormal levels.

Why would a doctor recommend an Alkaline Phosphatase Test?

A doctor may request an ALP test if you are experiencing symptoms such as fatigue, jaundice, unexplained weight loss, bone pain or digestive discomfort. It can help identify liver disease, bile duct obstruction or bone disorders such as Paget’s disease or osteoporosis. Because alkaline phosphatase levels can be affected by multiple systems in the body, the test is often used alongside other blood tests to build a clearer clinical picture.

What does a low alkaline phosphatase level mean?

Low ALP levels are less common but can sometimes be associated with conditions such as malnutrition, zinc deficiency, hypothyroidism or certain genetic disorders. In some cases, persistently low levels may indicate impaired liver function or issues related to bone metabolism. Interpreting low ALP results usually involves looking at other blood panel findings to determine the broader context.

What causes high alkaline phosphatase levels?

Elevated ALP levels can be a sign of liver disease, bile duct problems, or active bone growth or damage. In children and adolescents, higher levels are often normal due to bone development. In adults, high levels may be linked to conditions such as hepatitis, gallstones, liver cancer or bone disorders. Pregnancy can also temporarily raise ALP levels due to placental production of the enzyme.
